Grow and Learn Speech Therapy Inc.

If you have called and made an appointment at Grow and Learn Speech Therapy, we will need to take care of some business before you come.

1.  Call your pediatrician and obtain a prescription or referral for a speech therapy evaluation.  Most insurance companies require speech therapy to be "medically necessary".

2.  Download and read our HIPAA (Health Information Portability and Accountability Act) information.  We will need you to sign that you have read and understand this document. 

3.  Download, complete and sign our "Patient Information" form.  Fax or send it back to us with a copy of your insurance card (front and back).  We will use this information to create a file on your child and verify insurance information and coverage before your visit.  We are an In Network (PPO) Provider for Aetna and Blue Cross Blue Shield.

4.  If you need for us to share information on your child with their school, doctor, or other professional, please complete the "Consent for Exchange of Information".

What to tell your Child?

Tell your child that they are going to come see "Miss Susan" and that she will have pictures and books to look at.  We will also play games and take breaks as needed.  If your child is very shy, they may want to bring a special toy or book to show the therapist as an "ice-breaker".

Parents are welcome to sit in on evaluations, listen from the waiting room, or leave and return to pick up their child.  We will decide what is the best situation for your child.
